Social Care Outreach

Social workers at Hope will often visit a young person and their family at home or in the local community in order to meet in a less formal surrounding and to aid engagement with the service. A young person may been seen on several occasions outside of the Hope setting in order to support them in accessing the Hope day programme or to continue to engage with another community provision.

Outreach visits to the home may also be used in order to support the family with issues such as communication, boundary setting and developing confidence in parenting skills in order to prevent placement breakdown. The amount of outreach with young people and their family can be flexible and it is always needs led.
